clinic Vendor & Sponsorship

There are two documents associated with Vendors wishing to sign-up for the OHSFSCA Clinic.

The first contains general instructions and information, the second contains the application that must be filled-out and sent to the Exhibit coordinator.

Both documents should be downloaded and read.

The Application Document (titled "Vendor Application Form 2025") needs to be filled-out and sent to the OHSFSCA Coordinator, along with a check and business card. The Coordinator's contact information is included at the bottom of both documents.
